Summary of Insurance
This summary is designed to outline the benefits of the coverage provided
by the Sure-Ski Season Pass Insurance Program and does not create or confer any
contractual or other rights. All rights with respect to the benefits of
an insured will be governed solely by the group master policy #100001557 issued by
Industrial-Alliance Pacific Life Insurance Company. A complete copy of this policy
is available from MHC Financial Services, Inc.
Summary of Coverage
Industrial Alliance Pacific Insurance Company and Financial Services Inc. (hereinafter called "The
Company") will pay, subject to the exceptions, limitations and other provisions
of this insurance, the benefits specified below. This coverage shall commence on
the date the premium is paid, and shall terminate at the conclusion of the current
ski season.
Benefits
The Company will pay non-refundable Season Pass charges according to the
schedule below in the event that an injury, sickness or death of an Insured
prevents participation in the sport for at least 30 consecutive days within
the ski season.
Refund Schedule
1. Season Pass refund benefit if the non-participation commences:

a) within 30 days of the official opening
|

b) 31 to 60 days of the official opening

c) 61 to 90 days of the official opening
|
- |
40% refund
|
|
d) 91 to 120 days of the official opening
|
- |
20% refund
|
2. Family Protection Refund Benefit:
If a parent, spouse (including common-law) or dependent child, who is also
insured elects to cancel their Season Pass. The Company will pay the
Season Pass Refund Benefit to the parent, spouse or dependent child
provided:
(a) benefits under the part titled Season Pass Refund Benefit was
payable in respect to the Insured Person;
(b) cancellation and surrender of the parent's, spouse's or
dependent child's Season Pass is within 15 days
of the Season Pass Refund Benefit being paid in respect to
the Insured Person. This benefit is limited to two (2)
additional claimants.
3. Job Transfer Protection
If the Insured is subject to a job transfer to a new city by his/her
immediate employer, with whom he/she was employed at the time
of purchase of the insurance, which necessitates the relocation
of his/her principal residence outside a 200km radius from the ski
area at which he/she is a Season Pass Holder, provided that the date
of transfer is at least 45 days after the purchase of the Season Pass
and the Company is advised within 15 days of notification of the
Insured by the employer.
Conditions
Insurance must be purchased concurrently with the Season Pass.
Exclusions and Limitations
The insurance does not cover loss resulting from or contributed to by:
| a) | declared or undeclared war or any act thereof; |
| b) | active full-time service in the armed forces of any country; |
| c) | suicide or any attempt thereat or intentionally self-inflicted
Injury, while sane or insane; |
| d) | mental or emotional disorders, unless hospitalized; |
| e) | pregnancy or any complications thereof; |
| f ) | a pre-existing or related condition whereby you received
medical treatment or required the use of medication
during the six months preceding the purchase date of
the Season Pass; |
This exclusion shall not apply to an Insured Person whose
treatment was deemed, by the treating Physician or health care provider,
as a routine follow up examination, nor shall it apply to an Insured
Person whereby their use of medication is for a controlled and medically
Supervised Condition, which was not medically compromised and whereby there
was no change in either the medication or in frequency and usage, or dosage
within the six months preceding the purchase date of the Season Pass.
Definitions
"Dependent Child" means any natural child, step-child, or legally adopted
child of the Insured Person, who receives support and maintenance from
the Insured Person and is under 21 years of age and unmarried, including
a child of the Insured Person's Spouse who is in the care, custody
and control of the Insured Person and living in a parent-child relationship
with the Insured Person.
"Injury" whenever used in this policy means bodily injury caused by an
accident occurring while this policy is in force as to the Insured Person
whose injury is the basis of claim and resulting directly and independently
of all other causes in loss covered by this policy.
"Sickness" means Sickness or disease, which causes a loss,
covered by the policy, provided such Sickness or disease
commences while the policy is in force as to the Insured
Person whose loss is the basis of a claim.
"Ski Season" means the period of time commencing with the official
opening day and ending with the official closing day
of the ski resort for which the Season Pass was purchased.
"Season Pass" means a Season Pass purchased for the purpose of skiing
or snowboarding at the ski resort.
"Supervised Condition" means a diagnosed condition that requires consistent
medical treatment. The same condition must not have required a
change in treatment and/or medication during the six-month period
immediately preceding the Season Pass purchase date.
