Report a Claim

You have two ways to report a claim with respect to your Sure-Ski Season Pass Insurance:

1)  print the online claims form (also available in French), complete it and return it to the Claims Department of Industrial Alliance Pacific Insurance and Financial Servies Inc. at 2165 West Broadway, Vancouver, BC, V6K 4N5 within 45 days of continuous non-participation;

or
2)  call and report the claim directly to Industrial Alliance Pacific Insurance and Financial Servies Inc. at 1-800-665-4458.

Please note that you must advise Industrial Alliance Pacific Insurance and Financial Servies Inc., of your claim for benefits within 45 days of you being unable to ski due to a covered condition.
